ContinueCARE Hospital is a 35-bed long-term acute care hospital or “LTACH,” located in the heart of Columbia. As a separate, fully licensed acute care hospital, we provide intensive and complex medical treatment designed specifically to meet the needs of the chronically and critically ill patient.
We uphold a tradition of providing compassionate and patient-centered care coupled with the latest in acute care interventions, technologies, and comprehensive therapies. We are located on the seventh floor of the Prisma Health Baptist Hospital.
As the only LTACH in the area and as a hospital-within-a-hospital, ContinueCARE Hospital enjoys the best of both worlds by having a relationship with a larger hospital, Prisma Health Baptist Hospital, yet maintaining a small-hospital atmosphere. Since our patients typically have a longer length of stay, our staff has the opportunity to get to know patients and their families. We believe spending quality time with our patients makes a difference.
Every patient has unique needs. A multidisciplinary health care team works collaboratively on care goals for each patient, developing an individualized, integrated program combining medical and therapeutic care. We educate both patients and families on what to expect, discuss goals, and involve family members in the treatment planning.
Our medical director, Mark Mayson, M.D., is a board certified pulmonologist with extensive critical care experience who leads a dedicated team that includes physicians, nurses, clinical liaisons, case managers, pharmacists, respiratory therapists, rehabilitation therapists, radiologists, and dietitians. “We have on staff at CCH a full complement of experienced hospitalists and subspecialists to provide comprehensive care for even the most complex patients. Our goal is to treat each patient from a multidisciplinary perspective to achieve optimal outcomes following best practices,” Dr. Mayson says.
We recently welcomed our new chief nursing officer, Michele Urban, whose nursing background includes caring for patients in the ICU and other specialty care environments. “My goal is to provide the highest level of care and comfort possible and to help restore health and vitality for our patients,” Urban says. Prior to joining ContinueCARE Hospital earlier this year, she served as chief nursing officer at a large acute care hospital in Connecticut.
“Most patients and their families are not familiar with the LTACH level of care until they find themselves facing a serious medical situation,” says Wayne Boutwell, chief executive officer. “We strive to be the clinical bridge that helps the patient progress clinically from the ICU to specialty care or to home. Many of our patients are discharged to their home with home health services. Being the only LTACH in the Columbia market, we take pride in the service we provide to the community, and we keep patient safety at the forefront of everything we do.”
ContinueCARE Hospital offers a wide range of specialized services, including a pulmonary program, medically complex program, and wound care. A major focus of the pulmonary program is weaning patients off a ventilator while also treating any additional injuries or complications. The medically complex program focuses on treating patients with heart disease, serious infections, uncontrolled diabetes, and many other conditions requiring intensive or acute care. Wound care management is for patients with pressure ulcers and other serious wounds. Often, our wound care patients have complex medical issues such as diabetes or vascular disease.
